There's a version of the agentic future where automation is the goal and human intervention is the failure mode — every step that requires a person to review, approve, or correct is friction to be eliminated. This is the wrong frame, and it produces brittle systems.

Human oversight is a design pattern, not a temporary workaround until the models get better. Some decisions should require a human. Not because the agent can't make them — often it can, with reasonable accuracy — but because the consequences of getting them wrong are high enough that the cost of a human checkpoint is worth paying. Sending an email to a thousand customers. Modifying a production database. Executing a financial transaction. The agent might get these right 98% of the time. The 2% is the reason you keep a human in the loop.

The more interesting question is where to put the human. Early in a workflow, you can catch bad inputs before they propagate. Late in a workflow, you can review outputs before they become real-world effects. In the middle, you can intervene on specific decision points — the ones where the agent's confidence is low, or the stakes are high, or both. Each placement has different costs and different failure characteristics. The design decision is choosing which combination matches your risk tolerance.

Teams that resist human-in-the-loop design often justify it with velocity — review steps slow things down, automation is the point, users want instant results. These are real constraints. They're also often overstated. Users don't want instant results as much as they want correct results. An agent that acts immediately and wrongly is worse than one that pauses and asks. The pause feels like friction until the alternative is an apology email.

The more autonomous your agent, the more important your human checkpoints become — not fewer. Full autonomy is appropriate for narrow, well-understood, low-stakes, reversible tasks. Everything else deserves a checkpoint somewhere.

Build the checkpoints first. Remove them deliberately, one at a time, as you earn confidence. The developers who go the other direction — who automate first and add oversight after something goes wrong — are always adding it under pressure, which is the worst time to make good design decisions.

Autonomy is earned. Oversight is how you earn it.
